Q: How can I receive an NSA Membership Application?
A: To receive an NSA Membership Application, choose from the following three options:
- Complete the on-line NSA Membership Application Request Form shown below. We will send you an application by mail. You can then complete the application and submit it by mail to NSA Headquarters at the address listed below.
- View the NSA Membership Application in Adobe Acrobat PDF Format. You can then print the application, complete it and submit it by mail to NSA Headquarters at the address listed below.

- Call NSA Headquarters at (480) 968-2552, Monday - Friday, 8 am - 5 pm, Mountain Standard Time, to request an application. We will send the application to you by mail. You can then complete the application and submit it by mail to NSA Headquarters at the address listed below.

To become a Professional Member of NSA, you must document that you meet any one of
these three options:
Option One:
Have you received monetary payment for at least 20 presentations within the
last 12 months?
If so, you are qualified! Send in supporting documentation now!
Supporting documentation for Option One can be any of the following:
contracts, paid invoices, check copies and tax returns showing speaking income. Written testimonials are not an acceptable form of supporting documention.
Have you given at least 20 presentations to audiences of 15 or more as part
of a salaried position within the last 12 months? If so, you are qualified! Send
in supporting documentation now!
Supporting documentation for Option Two is a letter from your employer
indicating professional speaking presentations that you have given in the last
12 months.

Please use the following Word Document template as a guide when submitting documentation. Using this template will help you and your supervisor to provide key information when submitting a letter.

Send in your letter written by your employer. Send with your NSA Member Application to NSA/Membership/1500
